In confirmation of my registration as a third-party and community fundraiser for Lung Foundation Australia, I am acknowledging that my participation may involve certain risks which may occur over the course of organising such fundraising activities. My participation and related activities are undertaken in a voluntary capacity, with a sound understanding and awareness of the risks that may be involved in the activities coordinated by myself.
I agree to waive all claims against Lung Foundation Australia and will not affiliate any liability, both directly or indirectly, for any loss, damage, claim, demand, suit or proceeding, or expense associated with injury, harm and death, against any personnel associated with Lung Foundation Australia including, staff/employees, volunteers and/or members, and The Board.
I agree to conduct all fundraising activities in accordance with Lung Foundation Australia’s Fundraising Guidelines, as well as complying to any relevant Local, State and Federal legislation.
I am confirming that I am eighteen years of age, or I have parental/guardian consent and permission to participate in the registered activity.
Lung Foundation Australia will use your personal information in accordance with our Privacy Policy or as the law permits. For the purpose of Marketing, including direct marketing (emails, phone calls, direct mailing), as well as through all social media platforms, we will use the information such as name, image and voice, to support your participation in the registered fundraising activity and/or campaign, as well as future related content, to inform you of information and campaigns related to Lung Foundation Australia. You can unsubscribe or update your preferences regarding how you are communicated with at any time by contacting Lung Foundation Australia.
